As of recording the Xbox achievements are extremely buggy for everything except the 3 star ones. In my experience some are flat out tied to wrong levels/medals and some are just buggy and require redoing. To be able to redo medals you need to close out the game if you do not get the achievement (DO NOT BACK OUT OF THE LEVEL OR THEY WILL SAVE) For the buggy ones it seemed to me that saving the last one you pick up as one BEFORE a checkpoint yielded more success (but still not guarenteed) To do so, pick up the later 2 medals and then back out so it saves them then pick up the first one. The Hub level ones seemed to me like they were more buggy if you picked up any on your way through the game so I would advise trying those after the story is complete (and why i put them at the end of this video) However, as I mentioned, some seem to just be tied to the wrong levels (as you will have seen in this video, especially in the tail end of the game). So my advice for how to go about it (outside of waiting for a fix to make them function properly) is to play through the game doing 100% and see what pops. For your missing achievements start a new save slot, beat the levels you need to to WITHOUT touching a single medal. Then proceed to do the method of retrying mentioned above (closing the game out when something doesnt pop) and keep at it until you have the rest. None of this is guarenteed, some might work for you, some might not. Keep at it and keep retrying and you should be able to get them all the unlock eventually. I did and so have others.
